2021 grant application
Information that you need to assemble BEFORE making the online application:
The Name of Your Project: this is the main identifier for your application that you will use on all of your communication with us.
Your organization: name, name of leadership, street address, email address, telephone number, mission/goals, website address - if available, number of total staff (both full time and part time), number of total seasonal employees/interns/volunteers, year the organization started, size of last year's budget, fiscal year that your organization uses, taxpayer identification number given by the Internal Revenue Service. If your organization does not have one you will need to furnish the name of the fiduciary agent that will accept our grant money on your behalf as well as its address, leader, email and telephone contact information. The fiduciary agent must have a nonprofit designation (501 c 3) given by the Internal Revenue Service. Contact us if you need additional information regarding this at [email protected].

- Please provide a broad outline of your project and what you expect to complete over the next year. In general terms, what do you plan to do? (200 word limit)
- How will you carry out your project? What specifically will your organization do? (150 word limit) You can use bullet points or just list your planned activities.
- What are your objectives for this project? What do you hope to accomplish? (100 word limit)
- How will you measure success for your project? (100 word limit) These are your outcomes / results -how you will show or tell us about how your project affected the lives of the people in the community that you worked in. Please consult our page "step by step guide to our application for more information.
- Who do you plan to reach with your project? select all options that apply. If you select "other" please identify them in your answer to the next question.
- In what other ways can you describe the people that you want to reach with your project? (100 word limit) Include percentages of people of color, those living below poverty level, those that speak a language other than English at home or any category that you would like to include that is relevant to your project.
- Please list the organizations that you are collaborating with for this project (100 word limit) Include each organization and their role in the project
- How are you going to ensure accountability to and representation from the community that this project will serve? For example, how are community stakeholders involved with your organization and in the decision-making for this project? (150 word limit)
- Describe any specific challenges and opportunities your group faces in accessing resources for your work (150 word limit) These can range from skills and capacity to group's tax status and competition for local resources. Please give us some insight into the group's specific context.
